July 8, 2009


Commissioners Present: Commissioner Sorensen, Commissioner Solomon, Commissioner Albee

Meeting called to order at 8:15 am by Commissioner Sorensen

Public Input:
1. David Babson: Right to know law. Made a settlement of $15k with Dennis Robinson, who paid for Dennis’s Salary? Was it the taxpayers or insurance?
2. Commissioner Albee: It was paid by line item in budget. The tax payer paid for it.
3. David Babson: What is going on with the Co-op extension? Any update on building? What is going on with old nursing home?
4. Commissioner Albee: Waiting to hear from BPS, they have schematic from architect. They are working up pricing.
5. Commissioner Sorensen: On July 20th we will be going to the delegation to have more money released.
6. David Babson: Why is Co-op extension money going toward developing this plan? Why upfront cost? Paying for decision on whether to utilize old nursing home or build new.
7. Commissioner Albee: Have to pay for architect plans up front, money would come from that budget in either case.
8. Joe Kowalski: Concern with nursing home article in Daily Sun yesterday. The talk about closing the nursing home is troublesome. Seems like delegation wants to pull the rug out from under home.
9. Commissioner Sorensen: New members of delegation are interpreting the replacement of nursing home differently. They feel that residents could be “farmed out” instead of building a new home and that would satisfy the agreement. Meeting on July 20th, hope room is filled with people who are concerned about this issue.
10. Commissioner Albee: Delegation wants to revisit the notion of not having a nursing home. Disturbing that Betsey Patten would come to public meeting and announce that delegation is considering closing home. No statutory obligation that county must have a nursing home.
Talk of closing the home, cost factor, low morale, residents displaced, and unemployment problem by town if home closes. Find out where our employees live. County is 100% liable to pay unemployment and Cobra. Discussion of study to see if there was any interest by others to take over nursing home. No interest at all.
11. Commissioner Sorensen: Effingham meeting, one Selectman used to work in Maintenance at home knows we need one. Other Selectman questioned why we are in the business. A lot of time has gone into providing data to delegation. Commissioner Albee: Have to look at local impact to government for unemployment if home closes. How is that reasonable? Look at costs.
12. Discussion of costs, taxes, payroll, etc. Have not spent time and rational thinking on this issue. No professional Administrator in Carroll County, asking for us to create economic models for them and we are not professionals in this area. Building committee came up with this design. Promote new nursing home. Simple majority- going to go forward
13. Joe Kowalski: Will be showing tape every week to promote building.
14. Rep. Susan Wiley: Have been reading my emails after the article in the Sun and Independent ran. Outraged that Head of delegation could come here and even suggest closing the nursing home. Existing home is not up to standards, we have an obligation to the people, can’t even talk about closing home. Have to stop this nonsense. Delegation has talked about promoting home care but we cannot close the nursing home. Must move forward.
15. David Babson: Do not believe that people realize just how many people the nursing home employees. 207 people would be put out of work. Big issue, economic impact. I will promote the number of people that you have working there.
16. Susan Wiley: Question on number of inmates and population. Question on payment to Dennis Robinson.
17. Commissioner Sorensen: Possibly down due to education/training programs. D. Robinson payment was in budget.

Regular Session

Commissioner Solomon made motion to approve sealed minutes of July 1, 2009 pursuant to RSA91A; 3 for personnel. Seconded by Commissioner Albee. Commissioners Albee, Sorensen, and Solomon in favor. So voted

Public minutes from July 1, 2009 will be reviewed next week
1. Commissioner Albee: Last nights’ meeting was productive. 2 Selectmen had so much resistance at beginning and by end of night we could answer each point of resistance and at the end they came around. Cost was put in perspective and explained. First debt payment is not due until 2011. Recession has bottomed out. 8 or 9 people from the public attended Effingham meeting. Need to do Madison, Tamworth, Ossipee, Sandwich. First time Commissioners have attended a selectmen’s’ meeting. Getting word out.
2. Commissioner Albee: Quotes from electric providers. UMG-NH and Patriot Energy Group. They sell bulk energy. Looks like UMG rate/savings is higher. Patriot will be calling back later today. UMG gives better terms. Will follow up with UMG and get more info.
3. Commissioner Solomon: Presented State flag given by Rep. Buco


Will Dewitte, Farm
1. Ham Roofing coming tomorrow to put roof on water tank. 48 cords are cut, split, and stacked. Replanted gardens from damage by water and geese. Baled hay cut on the 4th is already sold. Metal trailer is gone, waiting for him to bring it back.
2. Commissioner Sorensen: Need receipts from him, we get half value of whatever metal brings.
3. Quote from Lakes Region Paving for Administration parking lot. Est. Total $2,400.
Commissioner Solomon will check on another company. Will is going to dig out, repack and compact. Need to meet to discuss plow truck- Commissioner Albee back in on Friday.
Contact chipping companies about chips for blueberries.
Looked at site, what is needed to prepare ground. 1000 plants ordered for spring.

Corporal Fowler, HOC
1. Current population is 54 in house, 4 weekenders, 6 transfers, 2 electronic monitor and 23 pre-trial releases. Total population 89.
2. Total number of admitted inmates for the week was 19 (11 repeaters) and total number released was 18. Repeater charges: 2 DWI, 1 Restraining order violation, 1 drug/alcohol related, 1 Disorderly conduct/resisting arrest, 1 burglary, 2 BOB, 2 OAS, 1 Criminal Threatening, 4 protective custody.
3. Farm has been taking out on average 4 inmates per day. Eligible inmates = 16
4. Primex has reviewed the Classification Policy. It has also been reviewed by Robin Gordon. No issues found. Would like Commissioners to review and approve policy.
5. Trane has started working on the A/C unit.
6. Bo Watson will be in to fix lighting contact and finish lighting issue.
7. One inmate on pre-trial release motioned the court to return to jail due to residence issues. Court approved motion, inmate is back in custody.
8. Repeater % is up 3% from last year. 2008 = 43% 2009 = 46% YTD
9. Admitted state prisoner/admin transfer
10. ITI/DSI was in to fix a blown board on the phone server.

4. Capital Equipment purchase Approval: 2 Canopies – budgeted $778 (unable to obtain 3 bids) Requesting $778. (for wheelchair gliders) Approved by Commissioners.
5. Certificate of Need (CON) Meeting with Susan Palmer Terry @ 12:30pm today. Working with John Lyford on budget.
6. Question on Café workers by Commissioner Albee. Will take them out of equation.

7. Commissioner Sorensen: Ask Paula to find out where our employees are from.
8. Commissioner Albee: How much work is done at your office in regard to Medicaid reimbursement and payments? How much is done with DEAS stuff. (Staffing requirement info needed for delegations figures.)
9. Contracts: In April Lakes Region Linen were bought out by General Linen Services. No other linen service option in the area. New Contract will yield an annual increase of $3,099.41. New contract is for 5 years. Would like to see renewal in 2 years. We can opt out in 90 days with notice if we build new building.
10. Commissioner Albee: What are the escalation costs in these 5 years?
11. Sandi and Commissioner Albee: None, would like escalation of cost and prices written in contract and contract made for 2 years instead of 5.
12. High level of frustration, concern and activity regarding article in paper about closing home. Met with concerned residents and families to explain what is going on as well as the importance of the July 20th meeting.
13. Robin Reade: Union president asked that bring up this topic. Air of happiness not felt in a long time. Sandi has made a big difference in the home.

Ann Aiton, Registry of Deeds
1. Final on Land Exchange (2 acres for 10 acres) Commissioners need to sign land exchange forms for State. (Commissioners signed and dated papers)

Dave Meyers, Sheriff’s Department
1. Commissioner Sorensen: Congratulations on promotion to lieutenant.
2. Dave: List of impound lot, request from Commissioner Albee and Will. Need to get a motion from court in order to get rid of evidence or give it back to insurance company. Will find out status of cases regarding evidence.
3. No statutory requirement to impound cars for State Police, have done it for courtesy.
4. (Looked at drawing of what proposed impound lot will look like). Room for 24 vehicles, secure area.

Robin Reade, HR
1. All insurance changes for non-union have been done for July 1st. Union for jail voted against it. Flexible spending accounts will start August 1st; have papers for Commissioners to review and sign. Meet with Primex again on Wed. July 22nd at 11am to go over other options. Do you want to meet with union rep?
2. Commissioner Albee: Finish meeting with attorneys first and get them involved.
3. Robin: Claim activity from last year.

1. David Babson: Leary of 5 year contract that doesn’t have an escalating cost stated on linen/uniform contract. Straighten out issue of Sheriff uniforms and clothing. Why are we involved in impound – need to get out of it.
Discussion of why non-public is being used for talk about the grant writing and Emergency preparedness position and combining them. Non-public was not needed.

Commissioners Present: Commissioner Sorensen, Commissioner Solomon, Commissioner Albee

8:15 Commissioner Sorensen called the meeting to order

Public Input:
1. Henry Spencer: Did Commissioners post the fact they will be at the Effingham Selectmen’s meeting. Yes it is on the website. Date is 7.7.09 at 7pm. Henry suggested Community Calendar in the Conway Daily Sun.
Asked about video monitoring at the Court – that is court budget and comes from the State.
2. David Babson: Commented on the fact that Dennis Robinson was paid $15,000.00 and wanted to know if the taxpayers paid that or was it the insurance company and why the minutes were sealed. He also said he was not able to get a copy of the employee manual to look at. Also asked if all 3 Commissioners would have signed the contract.
3. Commissioner Albee: We are limited in what we can say. The termination agreement has been made public but the minutes for non-public will remain sealed. You will have to go through the process of unsealing them in accordance with RSA 91-A. Commissioner Albee asked Robin R. to get David B. a copy of the manual.

All three Commissioners said they would have signed the contract, but there was only room for one signature and David Sorensen, the Commission Chairman, signed it.

Regular Session

Commissioner Sorensen made a motion to approve last week’s meeting minutes, Commissioner Solomon seconded. Commissioners Albee, Sorensen, and Solomon in favor. So voted

Commissioner’s Report:
1. Commissioner Sorensen went on to discuss blueberries. Will get in 1,000 plants per acre at the end of six years could bring in $12,000.00. Plants are $5.00 a piece and will be planted next spring, but need to be ordered now. Need to have $5,000.00 in budget for next year. From the Farm meeting that we had feels there is market and can turn a profit with these.
2. Commissioner Albee talked briefly about the Wolfeboro meeting and the Conway Selectmen’s meeting with the building project. He felt it was very productive and there were only a few naysayers.
3. Commissioner Albee read the email from Betsy Miller as follows:

State Treasurer Kathy Provencher has invited me to attend a meeting regarding the
Availability of the above bonds to NH counties. The Office of Energy and Planning
will also be represented.

The Energy Bonds are available to NH counties with populations over 100,000 (4
Counties?) and the Recovery Zone Bonds are allocated to all 10 counties.
This is a meeting to learn more about these opportunities and to lay out the options.

The meeting is scheduled for Wed., July 8th at 10:00am in the State Treasurer’s Office
in the State House Annex. PLEASE let me know if you want to join in attending. (the
location depends upon how many of you want to attend.)

4. Commissioner Sorensen asked that they take action on the blueberries.

Commissioner Sorensen made a motion that they order, receive and pay for the blueberries next spring. Commissioner Albee seconded the motion. Commissioners Albee, Sorensen, and Solomon in favor. So voted

5. Commissioner Albee had asked Paula to get a per bed figure from Merrimack County, Sandi would follow up on that.
6. Discussion on staffing efficiencies. Possibility of adult daycare how difficult will this be and will look at this. Sandi will get the state regulations for the Commissioners on this.
7. Commissioner Albee asked about establishing the rate of private pay and how long it takes them to become a Medicaid resident – he is looking for the last 5 years of data on this.

Jason Johnson, HOC
Current Population in house is 54. 3 weekenders, 2 elec. Monitor, 6 transfers, 23 on pre-trial release. Total of 88. Admitted 21, 11 repeaters, released 19. Repeater charges: 3 OAS, 2 VOP, 2 Warrants, 2 Assault, 1 DWI, 1 PC. Farm has been taking average of 4 inmates per day, 8 eligible.
1. Ordered gas meter for jail will be here in 4 weeks.
2. Commissioner Albee asked about the status of the computer lease. Robin G. is reviewing revised classification policy and Jason is waiting for feedback.
3. Jason will meet with Rob H. and Will D. regarding utilizing the jail kitchen.
4. Commissioner Albee: wanted to go on record as offering to use the farm as a non-profit to grow medical marijuana.
5. Have not heard back from Primex in regards to the new Classification policy. The person contacted was on vacation.
6. Contacted LA Drew last week. They made an appointment to come in last Friday, however they never showed up. Joe Boyd is following up with them today as well as Seacoast Crane. Update: Joe made contact with Drew, they will be in tomorrow. Followed up with Seacoast Crane. Possibly cheaper to expand pad and move machines. Waiting on hearing from Trane. Sketches done of roof, estimate not complete yet.
7. We have new uniforms that are more appropriate attire.
8. Omni-Care has not sent contract for review.
9. An inmate that was being held for NH State Prison was released by the court yesterday.
10. Inmate on Trial last week for drug possession was found Not Guilty by jury.
11. FYI number this year: inmates booked are 477 in comparison to 394 this time last year. Up 83 inmates.

Focusing on “what makes sense” (duplication of Effort, Efficiencies) Staff smoking and
Cell phone use will be addressed as well as professional behavior.

Discussion on number of employees needed for new home staffing, how many additional? Keep Café separate from other departments, show as rental w/income.
Restructuring maintenance schedule. Discussion of perspective residents who are willing to come if private rooms are available, but do not want to share room or bath.

Talk about statistical analysis of future in regard to nursing home. Impossible to know.

Betsey Patten, Joe Fleck: Delegation Representatives
1. Betsey: Looking at basically concrete number of $23 million for new home. What we need to have is all figures on the table. Getting down to nitty gritty. Hit on economy.
Want issues and numbers out on the table in order to make the decision. Not sure of options. It will either be $23.8 million or get out of the business.
2. Commissioner Albee: If we wait we will lose interest rate. Get bids out while recession is on and we get the best prices and rates. Bond schedule shows payments and when they are.
3. Betsey: Man in Wolfeboro said “Huggins hospital issue” 8 or 9 months of delay cost much money. Looking at delays that cost money. Time line. Looking at payment schedule.
If we decide to get out of business, what is the amount that we still have to come up with?
4. Commissioner Albee: If we get out of the business it is not going to be free. We already have $150k invested. Time to relocate residents; do something with building, contracts and termination issues. Looked at selling of beds, difficult. Must go before CON board and must approve of price. CON board may take the beds and allocate them.
5. Joe Fleck: No doubt we need a new nursing home. Better to go forward with best design that we can afford. Want quality. Concern is if we go forward with that design, should we even be in the nursing home business. Would like to see as much information as possible on where our residents could be placed.
6. Commissioner Albee: Significant number of Carroll County residents would not be able to stay in the county. Not enough beds in Carroll County.
7. Commissioner Solomon: Private homes do not have to take these residents. Can make excuses.
8. Betsey: Need to justify the need for the $300k. There is concern out there about the price and if we should be in the nursing home business. We will make the judgment call on July 20th. I do not want to get in to a judgment call when we are bringing the numbers out. Cannot say there is no interest. Need to know what the process if about moving residents. Need the answers to all the questions. I understand we are the third largest employer in County. Reality is hitting. Wish we had done this with the jail.
9. Commissioner Albee: Problem when looking for that type of information is the employees. They panic and then feel that the nursing home is going to be closing. We have brought a lot of information to the table and will bring more.
10. Commissioner Sorensen: Rep Chandler and Butler made recommendation last July to replace nursing home. Are they going to go back on this now that we have the proposal? That is an issue. The motion was made.
11. Joe: Replacing the nursing home may have different options. Moving residents may be an option.
12. Commissioner Albee: Have to look at how this impacts staff, how it impacts residents.
13. Betsey: Would be remiss if I did not bring this up to you. Staff has a lot of angst about delegation making a decision. I want to be able to have answers. What will it do to the system if we shut it all down and move all the people? How many will it affect?
Medicare patients have no choices
14. Commissioner Albee: Now we have to look at how we are going to feed our prisoners. We will have to address that issue if we do not have a nursing home. Must look at calculations but will be an estimate only. Can roughly calculate cost of feeding prisoners now. How much will a food service company charge? Look at the business standpoint. County is in the public service business, must take care of our residents. Must look at the personal side of nursing home. Financial constraints would have to dramatically outweigh the service. Getting out of the nursing home business would be difficult.
15. Betsey: point out issues that would arise if we get out of nursing home business. I need different views, costs, system, employees based on what you have told me.

Discussion of past decisions and studies done. Original projected cost for new home based on Pulling study. Bank rates, Bond markets, Looking at process of running county, act of management still continues. Everything must be on the table to make a good decision on what we are going to do, and feel good about the decision. Where do we go from here?

Discussion on pellet boilers and availability of pellets in this area. Size of boiler, creation of premium pellets. Pellets that Propell is using are manufactured by Main Energy Systems. Propell is actually owned by New England Wood Pellet in Jaffrey, NH.
Propell does Commercial/Industrial boilers. Wood pellet prices VS. oil and propane prices.

Need calculations of energy savings by switching to bio-mass energy from oil/propane. Does the pellet boiler become less efficient in the summer when heat is not used but hot water is?

Explanation of modulating boilers.
We would sell equipment to Carroll County; you buy your fuel from wherever you want to. You will be responsible for service as well.
Systems are 100% automated
Simple training of facilities people. Check system once a month and clean once a year.
Repairs? Per year approximately $1000 in repairs, etc.
Description of systems, silos, delivery system, etc.
Electrical costs to run system.

Delegation Public Notice

CARROLL COUNTY


The Carroll County Delegation will meet in Executive Committee on Monday, July 20, 2009 at 9:00 a.m. to meet regarding Mountain View Nursing Home Project, the U.N.H. Cooperative Extension, and the Capital Reserve.

The meeting will take place in the Carroll County Administration Building, Delegation Room, at 95 Water Village Road, Route 171, Ossipee, New Hampshire.

The Delegation also will consider and act upon any other business that may properly be brought before the meeting or any adjournment thereof. All citizens are invited to attend and ask questions. If you need any specific accommodations, please contact us at the Carroll County Business Office, 539-7751. (ADA)

The meeting of the Executive Committee will be followed by a meeting of the County Convention to vote on any recommendation or other business.

On Thursday, May 28, 2009 a letter of commendation was given to Officer Windy Kelly, from Captain Jason Johnson. Through the vigilance of Officer Kelly, drugs were confiscated. Captain Johnson expressed his sincere gratitude for your efforts to intercept contraband coming into the facility. Thank you for a job well done.

Mountain View Nursing Home Building Project

MOUNTAIN VIEW NURSING HOME
BUILDING PROJECT

1. Our current population of Medicaid recipients is used for all calculations and having no way of knowing what the population will be going forward, the number remains constant in all of the calculations. We know that short of a mass migration out of Carroll County, the population numbers will only increase, causing the County costs to rise;
2. Inflation/deflation will effect all options equally, so there has been no calculation for them;
3. Current operating costs and income rates have been taken from the County Budget;
4. Interest rates have been quoted by commercial lenders and the NH Bond Bank, (payment schedules attached);
5. Tax rates have been calculated from the current DRA schedule;
6. Medicaid rates and reimbursements have been obtained from the Department of Health and Human services;
7. No adjustments for paying off any other county debt have been calculated into the tax rate going forward. But as we finish paying off the Jail and other notes those reductions of debt payments will affect the County tax rate.

Option 1:
Replacing our present nursing home (40,000 sq./foot) with a similar but a more efficient model (55,000 sq./foot). Without giving any consideration to quality of life or other intangibles, the following assumptions can be made: a) the most substantial savings in operation will be in energy and maintenance; b) historically our population has been controlled by the room configuration and an average population of 97 residents will continue.

Option 1
1. 55,000 sq./foot nursing home construction costs: $15,000,000
2. Interest for 20 yr note 5%: 9,072,750
Total Cost $24,072,750
3. Medicaid capital reimbursement:
356,478 yr x 20 = - 7,129,560

Building a new nursing home to current market and design standards to optimize return on investment to the County. While we can never assume 100% occupancy, we can determine through our previous experience that we can increase our average population from 97 to 101 by having private and semi-private rooms. We can also increase our number of private paying (insured) residents and thus decrease our operating losses (although for this paper we have not adjusted our revenue to reflect this increase). The addition of a café will return revenue to the County.

Although option 2 requires an initial commitment of $8,560,000 more money, its increased Medicaid reimbursement and enhanced revenue options has reduced that difference to $1,570,230 by year 20. Going forward, by year 25 option 1 would have a gross cost of $70,918,915 and option 2 a cost of $70,880,395, making option 2 less expensive to build and operate.

CONSIDERATIONS
1. The County’s obligation to pay its portion of Medicaid will continue regardless of our actions. Our current DEAS payment is $3,709,980 which, if it were to remain constant, will be a $66,159,000 cost to the tax payers over twenty years. This amount represents .11/$1000 on our current tax bill.
2. Option 1 is premised upon a design which is similar but an improved model of what we currently operate and even though the initial cost is 36% less than Option 2, the 20 year costs are very similar.
3. Any additions to Option 1 to accommodate for the enhanced revenue possibilities of Option 2 would require spending more money and expanding the building’s footprint and reducing or eliminating any near term fiscal advantage it would have over Option 2.
4. For this example we have used a rental figure for a café in Option 2. If we were to operate the café ourselves and had gross sales of $450,000 with a 15% profit, our revenue would change from $21,600 per year to $67,500 per year. This $45,900 would translate into $918,000 over the 20 year’s life on the loan.
5. Any discussion of privatization would have to concentrate on what the commercial nursing home operator’s expectations are with regard to design and commercial viability. In order for the County to generate the most favorable deal for the tax payers, we need to have the most commercially viable facility.
6. Any consideration of closing the nursing home would have to include the fiscal impact to the County of its third largest employer reducing its workforce to that degree.

6. Commissioner Sorensen: We are in Conway at Selectmen’s meeting at 4pm on June 30th.

Jason Johnson, HOC
Current Population 53 in house, 4 weekenders, 2 electronic monitoring, 6 transfers, and 18 pre-trial releases for total of 85. Admitted 22 for the week, 11 repeaters. Released 26. Repeater charges: 3 for warrants, 1 VOP, 1 PC, 1 Assault, 1 UPA, 1 BOB, 1 DC, 1 Burglary, 1 DWI. Farm has been taking out average of 5 inmates, 16 eligible.
Officer Phillips completed his classification class. A revised Detainee Request Form has been drafted to help ascertain more information to help assess and classify the inmates. He has finished the new classification policy, had his instructor review and sign off on it, will be forwarded to Primex for them to review and then bring to commissioners for approval. Robin Gordon has already reviewed document.
Over next two days we will be releasing two inmates we had on Administrative transfer from NH State Prison. Both have made parole.

6. Jury coming in to view MSU regarding a case as a result of a search in MSU– will relocate inmates during that visit. Four officers will be testifying in that case over the course of today and tomorrow.
7. Discussion of Cyber-tron, equipment and leasing costs. This is the communications system. Jason and Commissioner Albee will discuss and arrange details later.

Will Dewitte, Farm
1. Updates: fixed step for ladder on dump truck. All wood is cut into 4 ft chunks to be processed. Taylor will deliver more when they have it. White truck is road worthy, inspected and registered. Looking at vehicle fleet plan. Recycled metal trailer owner will try to remove and dump it this week. Will then bring it back. 50/50 deal.
Jason coordinating with training of farm staff to deal with inmates.
2. Commissioner Albee: Jail will be instituting updated classification program.
3. Commissioner Sorensen: Need to do water samples for month of May. Will check on availability of blueberry plants for next year. 1000 plants per Acre, close to pond.
4. Commissioner Albee: Soil testing of fields; adjust soil now for next year.
5. Will: Blueberries and Christmas trees cannot be close together due to disease. Will check with Tina from Co-op forestry.

Mike and Kate Cauble, Carroll County Coalition of Emergency Preparedness
Discussion of how long the grant is good for (1 year, renewable for 2 more). Discussion of employee(s) required for position. Can it be incorporated into Sheriff Department position to move money through Sheriff Department? CCCEP focuses on public health, Sheriff is natural disaster. One purpose = Shelter. Determine flexibility of grant. Grant has been awarded and fairly assured of being received.
Discussion of interviewing and office space for CCCEP. Delegation will want to be part of setting up new department.
Further conversation needed. Possibly coordinate with Sheriff on position. Person must be approved by State.
Discussion of grants, areas, counties, emergency management, Health officer. Next meeting of Coalition is July 13th at 10am in delegation room. Caubles will be retiring as of June 30th. Discussion of anticipated return of Swine flu in the fall.

Kathy Garry, Business Office
1. New flag has been received, pole ordered for delegation room. Also working on getting State flag.

1. Followed up on capability of courts ability to do video arraignments. They do not have the capability. Jason is going to check on status of this at the jail.
2. Commissioner Albee: Sheriff is in process of hiring grant writer. Looking for grants to update communications.

Robin Reade, HR
1. Received email on Economic checklist – will forward to town, their responsibility. Review the Primex Medical Savings and other aspects of contracts/flexible spending (limit $1500). Union will be voting today. Nursing home/Jail. Already signed non-union contract package. Pertains to reworking deductible and co-pay plan.
2. Have had three replies from law firms (attorney to represent commissioners in negotiations with unions). Reviewing options. Start interviews on July 1st at 1pm.
3. Thank you to Commissioners for coming to employee recognition gathering.
4. Commissioner Sorensen suggested having some kind of committee where people can nominate a person who goes above and beyond in their job. Look at this concept.

Presentation on usability of wood fuels. How complex/nursing home can benefit.
Overview
No capital cost to facility
1. Give boiler, silo, enclosure, deliver pellets, remove ash, and monitor BTU’s. Monthly bill for BTU’s. No minimum requirement of usage. Runs parallel to existing system. 10 year commitment. Carbon offset credit belongs to Wood fuels
2. One system, must do analysis of distance and piping needs.
3. Cost of BTU’s is based on trend of fossil fuels slightly discounted. Ability to lock in depending on boiler and system. Paying for created BTU’s and BTU’s lost in circulation.
4. Piping is usually the responsibility of the complex. Possibility that we could do buildings in close proximity.
5. Developers will know projected BTU usage; I can do an initial analysis for complex.
6. Commissioner Albee: Benefits: Reduce cost, keep fuel local, and be green.
7. Drew: Modern, first metered systems are going in at Franklin Pierce. Contracted for 2 systems. State of Maine is also looking at contract for correctional facility. Local supply will be available by the time customers are set up.
8. Commissioner Albee: Once we get design we will look further into your process.
9. Commissioner Sorensen: Any benefit to having 2 plants?
10. Drew: More efficient to bring it all together with one boiler.
11. Henry Spencer: Cap on BTU charges?
12. Drew: Looking at $4 - $5. Wood fuels = safe transport, self contained system. Must get stats and data to put something together, potential design. Responsibility of customer for electric demand. Will have some info on cost of electric.
13. Babson: we are losing our power of negotiation by going with you.
14. Drew: no capital outlay. Yes, you are losing the negotiation but we are giving the equipment, maintaining it, and servicing it.
15. Commissioner Albee: Need to look at projected, unknown cost of energy and longevity.
16. Drew: Longevity is 20-30-40 years depending on maintenance.
17. Commissioner Sorensen: AT the end of 10 yrs we have option of purchase, continue, or remove.
18. Henry Spencer: What if your business model fails?
19. Commissioner Albee: would have to have wording/clause in contract if they go bankrupt.

21. Drew: slight particulate, lowered Co2, no smoke, no smell. All Permitting and testing is done by Wood Fuels. You can keep ash for fertilizer or we can take it. Your choice.
22. Commissioner Albee: differences between pellet and chip, chip usage, pollution, storage, waste. Chips are locally produced and controlled.
23. Drew: Wood chip delivery is inconsistent. 3 x wood chips in 1 pellet. Efficiency varies in chip- consistent in pellet.

1. David Babson: Co-op web page discussion concerning information on site, which site is not upgraded or informative. Commissioner Albee commented on the info listed on the State website for Co-op extension.
2. Commissioner Solomon: Concerning my column, send info from Co-op to me and I will put it in article. Column gets local news out to public concerning new meetings, upcoming events or things of interest to people in general. All town columnists should get information about what is going on and they can put it in their columns.
3. Ashley Rigazzo: Send to me, we can publish also. (Independent)
4. Babson: Gerry Menke requested information and Tina Savage sent the info to him on Co-op.
5. Commissioner Albee: Asked Gerry to come up with budget for re-doing old nursing home with offices, classrooms, etc.
Discussion of what uses and cost could be for old nursing home building and what is needed from Gerry
6. Henry Spencer: any movement on video arraignments?
7. Commissioner Albee: Have not had any. Must come back to table. Have asked Jason and John to find out if funds are still available. Transporting is an issue for security. Ultimately it will save money. Northern Court is set up for video arraignments.
8. Babson: This courthouse is not set up for it?

10. Maureen Spencer: Impound yard, looking at sites again? (making decision today)

11. Commissioner Sorensen: Getting meter for jail, have company coming to look at putting a roof over A/C unit. Insurance has been received for A/C damage.
12. Babson: Question on architect liability.
13. Commissioner Albee: Building Committee should have caught issue in design before build. We accepted design. Have to change design issues or put guarantee on it.
14. Babson: Hire architect to solve problems, they should have some responsibility. Laymen would not see those issues on a plan. What are you doing to tighten up issue for nursing home?
15. Commissioner Albee: Schematic/outline so far. Actual design is not done yet.
Discussion of design and issues that may arise. Also in new nursing home if building systems are designed improperly that is easier to follow through. Who decides if outside wall is not right? Until actual plans are drawn up, cannot make changes. Next building committee meeting is on June 29th.
16. Commissioner Sorensen: Meeting in Moultonborough Monday, lots of suggestions. Good support, turnout was better than Wakefield.

General Session

1. Commissioner Solomon: Co-op meeting: Wendy will be contacting Will (farm) on work that needs to be done. Master Gardner issue will be brought up again.
2. Commissioner Albee: June 22nd, employee recognition garden party. Pushing delegates to make a decision. If not done by August 31st we will lose any opportunity to get civil engineering and site work done as well as opportunity to have 5% money. Delegation needs to get the voting done.
3. Commissioner Sorensen: Pushing for decision/vote at July 13th delegation meeting. Chairperson is being leery about what is being presented and if delegation knows enough about building to be able to vote on it. State has 50 days for alteration of terrain. Will also hold things up.
4. Commissioner Albee: Permitting and civil is not in budget this year. Delegation is aware of time constraints.

1. Truck is back and runs/looks good. Concern about ladder design, explanation. Has 18ft body-long- dump body goes very high.
2. Meeting with Wendy on Timber harvesting, maple sugaring. Suggest give person only 1 yr contract for sap. Scheduling that area for timber harvest. Plant more sugar maples and grow them specifically for production, find out minimum number of taps needed. Need one more year to see availability and plan.
3. Called Taylors (wood) will fill two loads when available.
4. Commissioner Sorensen: Money can also be used to buy hay.
5. Will: Sold all feed hay already. Very little mulch hay left. All sold out of barn.
6. Looked at 1 ton plow trucks. Used $15-20k. Average of 50k miles. New = low $30’s.
7. Commissioner Albee: Suggest 1 ton dump.
8. Will: Visibility issue, do not need dump body on plow.
Discussion on plow truck, new VS. used. Warranty, use, maintenance, 2nd hand problems not needed. Goal for truck, negotiate. Ideal would be 1 ton, diesel, and plow, used for County business as well. Need plan on vehicle purchase for next year. Backhoe is depreciating as hours are put on it.
9. Commissioner Sorensen: If we have structure for hay wagon, can you work on it?
10. Will: White truck is almost useable.
11. Commissioner Albee: Looking at site today for impound yard and hopefully making final decision on placement.
12. Commissioner Sorensen: Need bigger lawnmower, look into it.

2. Total number admitted was 34 (9 repeaters) total released was 34. Repeater charges: 3 warrants, 2 violation of probation, 1 Protective custody, 1 resisting arrest, 1 admin transfer, 1 aggravated DWI. Farm has been taking out average of 3 inmates per day, 9 eligible.
3. Took 1 inmate in on administrative transfer from NH State prison. He is sentenced inmate and classified as minimum security.
4. Officer Phillips is in the classification class. Began Monday and schedule to finish today at 2pm.
5. Met with Will last Thursday to develop policies with farm. Currently have twelve policies drafted. Hope to have review next week. Also contacted NH State Prison and received a copy of the inmate evaluation form they use. Think this will work well with the new classification policies.
6. Contacted Primex and was able to make claim for A/C unit, check received for repairs. Seacoast Crane and Building was in yesterday to view back roof and provide estimate on putting roof over unit.
7. Held meeting in facility to discuss new health plan. No one showed up.

1. David Babson: Has Co-op material been reviewed?
2. Commissioner Sorensen: Yes
Discussion of build and design, proposal, request for bid component, request for quote, advertisement for hiring project coordinator and construction manager, etc. Mr. Babson will inquire of Co-op for more details.
3. Commissioner Sorensen: Commissioner Albee is looking at alternatives for Co-op building.
4. Commissioner Albee: Need to defend reasoning, cost, and be pro-active. Have to do math on justification of expense and how locals will benefit financially. Impact. Looking at web site for Co-op, not informational in regard to what Co-op does.
5. Lori Lenart: Co-op website is terrible, needs to be updated
6. David Babson: Shame to tear down all of the old nursing home, it has everything needed.
7. Commissioner Albee: Architect also feels the old home would be useable for Co-op’s needs. Take wings off and leave center portion. Save money. 13,000 sq ft in center of old building useful for many things.
8. David Babson: Paying rent in Conway of $50k - $60k per year is outrageous
9. Commissioner Albee: Cannot justify that cost.
10. Commissioner Sorensen: Look at cost of building. Problem: secretaries of UNH Co-op are technically county employees. If they are on the complex they are a liability of county. Right now they receive UNH benefits. Go back to Co-op and ask what possibility is of taking over space in old nursing home building.
11. Commissioner Albee: Look at numbers with architect.
12. Henry Spencer: Look at retrofit nursing home – money available if listed as emergency shelter. May be useful.

Out of public session, begin general meeting

Jason Johnson, HOC
Current population is 59 in house, 2 weekender, 1 electronic monitoring, 6 transfers, and 17 on pre-trial release for a total population of 85. Total number of admitted inmates for the week was 31 (15 repeaters) total number released, 27. Repeater charges: 2 DWI, 2 Assault, 1 Unlawful possession of alcohol, 2 breach of bail, 1 criminal mischief, 1 criminal trespass, 5 superior warrants, 1 protective custody, 1 burglary, 1 escape, 1 resisting arrest, 1 controlled drug charges, 1 endangering the welfare of a child. Farm has been taking out average of 5 inmates, 15 available.
Last Monday new digital antenna was put up. Digital converter boxes were installed in dayrooms on Tuesday. Inmates were advised to either have digital capable TV’s or have converter boxes sent to them.
Friday took in 1 inmate from Cumberland County Jail (ME). 2 confirmed cases of Swine Flu at ME facility. Inmate was taken by Sheriff’s Department to Huggins Hospital for flu test. Still waiting on results. Inmate is on isolation status, not showing any signs of flu.
Went to Norfolk County Jail with two other staff members. Took tour of facility to see operations. Spoke to instructor for Direct Supervision. Hope to have class in October and will talk to other Superintendents to see if their staff would be interested in training.
Spoke to Plato Learning about their program. Asked them to send a quote for services. We buy license for their services ($16,000 includes 3 computers and training of person who will supervise classes) and as many inmates as we have can go through the program. They also offer vocational and secondary education. Data can be exported and given to unemployment office, same testing systems. Will help inmates obtain employment.
1. Commissioner Albee: Look into stimulus/grant money for education, how many inmates have benefited in last 4 years, find out if other counties are using program and if benefit for multiple counties (discount).
2. Commissioner Sorensen: need to reduce cost of educational program by having this program take place. Stimulus only lasts 18 months.
What is the status on the A/C units?
3. Jason: Looking at putting a roof over them.

Will Dewitte, Farm
1. Commissioner Sorensen: Where is the truck? Garage has had it for 6 months. Will hold back $2k until truck is in our possession and working properly.
2. Will: Afraid they will not release truck without full payment.
3. Commissioner Albee: Find out about warranty/guarantee. If where is/as is then we have no choice.
4. Will: Three possible locations for impound yard. Bailed some hay this week, need hot, dry weather. Trying to come up with inmate inside programs for rainy days. Looking to build/buy hay wagons.
5. Commissioner Sorensen: Mr. Babson could you ask Mr. Olkkola.
6. Will: Trying to get white truck useable. Look at frame, cost to repair VS. buying used. Consider attending state auction.
7. Commissioner Sorensen: Maple syrup – thinking was 5 year lease but if farm can do it, we will not do extended lease.
8. Will: Must look to see if we have enough trees to make it worthwhile. Farm has nothing to do with front gardens at nursing home.

1. Census: 97, 1 death, 2 discharges, 1 admission. 1 admission pending Medical Director approval, 3 in process
2. Certificate of Need (CON) application has been filed. (gave copies to commissioners) Now need to review for completeness.
3. State plan of correction: Need for additional oxygen rack ($242.69), Anti-trip device needed for fire alarm. Panel is 40 years old, needs to be secured. Investigating working on Dutch doors- need to be planed and sized. (in kitchenettes, business office) Tops were removed, bottom not tight. Scheduling room needs door

New building, looking at space, rehab, kitchens, etc. Planning – looking at the future.
4. Commissioner Sorensen: Rehab for outpatients? Specialized? Separate entrance?
5. Looking at 1 household to have all the rehab residents. Building committee did not want competition to other rehabs in the area.

7. Employees would like to benefit also and use facility. Maximize what we have. Set schedules for outpatient and resident rehab.
8. Commissioner Sorensen: Push to have daytime rehab with outside entrance.
9. Commissioner Albee: What is perception of rehab dept. personnel with what they need?
10. Sandi: Work in an organized fashion. Will look into rehab, space, needs, scheduling.
11. Commissioner Albee: Talked to Gerry (EGA) about ability to add on/expand for rehab facility.
12. Sandi: Look at how efficient area is, do we need 2 activity areas. Must begin to work concept. Must increase revenue strain.
13. Commissioner Sorensen: Goal – increase revenue/decrease expense
14. Sandi: Name of home, something other than “Mountain View Nursing Home.” Want concept other than “nursing home.” Hair stylist is leaving, interviewing (contracted service.)
Discussion of rehab, shower rooms, value of having spa bath, benefits of having showers in each room (time and dignity issue).
15. Commissioner Sorensen: Calls received in support of Private rooms. OCC poll = private rooms.
16. Commissioner Albee: boils down to operational costs and staff for delegation. Hopefully can satisfy both.

1. Gave copy of memo that went out to employees about meeting with Primex on Monday. 2 sessions at HOC, 1 session at admin, multiple sessions at nursing home. Commissioners and Robin will attend some or all of sessions.
2. Scheduled next round of supervisory training over 5 months. Target line supervisors.
3. Robin and Stephanie are centralizing personnel files in database.
4. Letter to attorneys for commissioners to review (union attorney) will setup appointments and have non-public sessions with interested parties. Union reps will know as well.

1. Ann Aiton very helpful. Packet given to commissioners about property. Discussion of land exchange, deeds, approvals, signatures, monuments, letter of agreement on cutting timber. All changes to clerical mistakes and wording. Titles and letter of intent must be complete before town will approve. (Ann recommends signed copies of deeds and she will keep original)

Charlie Agnew, BioMass Commodities Corp, Williamstown, MA (Portland, ME)
Mr. Agnew presented an in-depth look at bio mass and the possibility of a central heating plant on the county complex. Also does feasibility studies for wood pellet manufacturing facilities.
Many facilities close by that deliver pellets and chips. Fiber supply is there locally. Base decision on scale of project. Equipment is most important.
Discussion of combustion systems, cost, step by step handout.
Trend: possible 50% increase in propane/oil costs in next 10 years.
Silo needed for storage of pellets (similar to grain industry) Threshold for acreage and BTU – air permit, emissions cleanup. Footprint 14x12x5. No smoke, 1 second every ½ hour puff of fine particulate only. Commissioners will go view boiler at Dartmouth.
Includes extensive training of staff member, full operation supervisor for 1 year.
Discussion of wood pellets VS. chips, manufacturer standards. Amount of pellets/chips that would be used based on past propane usage.
$200,000 estimate for 2.5 million BTU boiler installed. Includes storage system. (piping is not included, done by contractor of choice) Need to supply building and water.
Operating costs depend on options. ½ hour maintenance per week.

8. Went to Merrimack County to discuss classification and alternative sentencing.
9. Meeting with certified jail manager next week for training.
10. Commissioner Albee: A/C issue, TRANE no help. Placement of unit not appropriate. Issue of slab placement and piping.
11. Commissioner Sorensen: Create overhang to protect units from weather. Repair after issue addressed. $9000.

2. 2 LNA’s nominated for state award in Concord. Pat Labranche and Cheryl Vittum.
3. LNA students taking practical exam tomorrow, graduate Friday. One is staying on and others may become per diem if they would like.
4. Medicaid rate setting increase of $4.93 per day. In effect July 1st.
5. Nursing: night supervisor is retiring from position and moving to position of 11-7 charge nurse part time.
6. New facility meeting every Tuesday. Tom McCardle helping with kitchen design and setup for projected needs.
7. Commissioner Solomon: Next meeting in two weeks. Ideas looked good.
8. Sandi: would like café to be revenue producer, appropriate structure and size.
9. Commissioner Albee: Need to start making decisions on equipment pricing. Talked with Ed Butler on 501c3. Meeting with Susan Palmer Terry today.
10. Commissioner Sorensen: Any state survey results yet? (not yet)
11. Discussion of staff addition for new home. Adding 1 ½ people to staff, not adding nurse.
12. Commissioner Albee: Helpful-take one neighborhood and make a list of people who will work on that neighborhood. Compare with picture of wing. Show what we are doing now and what we will need to do.
13. Sandi: No additional FTE’s in nursing, 1 FTE in housekeeping related to floors, 3 FTE’s related to café, plan to augment with volunteers as well.
14. Commissioner Sorensen: Put into proposal what is needed, do not add later. Completion date of home is summer of 2011.
15. Sandi: Move in after June- will not get rate set until January. Would like to move in before (by April-May) to get increase in rate. (Tentative for May 1st now).

Commissioner Sorensen: Feds are going to change health insurance program. How will national health impact facility?

1. Commissioner Solomon: Spoke at OCC lunch, gave info on public meetings and where they would be held. Receptive audience. Need board/diagram to distribute to public places.
2. Commissioner Albee: Need smaller design board to distribute. Link on County web site.
3. Henry Spencer: Have Admin. Staff call Selectmen and towns and tell them when meetings are going to be held and where.
4. Commissioner Albee: Revisit loss of money even without nursing home. Cost $3.5 million without home.
*What is our actual obligation of County for cost of care? Will do further study and nail down figures. Build conservative structure that will return investment.
